The risk of bacterial penetration into implant-abutment connections

Placing implants and attaching abutments to the implants at the end of the healing period – are surgical procedures. The success of these procedures depend on a number of factors a dental surgeon should demonstrate the highest level of  professionalism and strict observance of all rules of aseptic and antiseptics, a patient should maintain proper, thorough oral care during the postoperative period and follow all the doctor’s instructions. Why is it so important to prevent infection when placing implants and abutments?

Implant infections are one of the complications of implantation

Implant failure rate of 1% is directly related to complications that arise in any infected area of soft tissue and bone around the implant or in the area of the implant-abutment connection. Although the quality of dental implants and the quality of their placement are getting better and they are becoming more affordable to patients, but complications still exist due to the lack of attention to the problems of infection. And this is especially true when the patient’s immune system is reduced because of chronic diseases or other reasons, such as age or lifestyle.

There are various types of both implants and implant-abutment connections. Dental implantologists, using them in practice, have concluded that implant- abutment connections are differently subjected to the microbial penetration as their connection tightness is rather different. It is also confirmed by scientific research conducted to study this matter. The fact is that if the connections are not tight enough, the occlusal load transmitted to the abutment is responsible for the formation of negative pressure between the implant and abutment, which causes penetration of liquid with microorganisms in the gap between them. These microorganisms then spread on soft gum tissues surrounding the implant and cause peri-implantitis. This complication occurs when surgeons ignore principles of aseptic and antiseptics when attaching the abutment to the implant. Periimplantitis leads to such complications as osseointegration failure, implant loosening, pain, swelling and even the necessity for removing the implant.

Thus, the tasks of dental implantologists, when placing implants and abutments, to achieve maximum sterility of the surgical field and all surfaces of micro-gap between the implant and abutments, to prevent the development of infection and teach patients how to properly take care for their implants. The proper application of antiseptics is required in all stages (preoperative and postoperative period and surgery). Antibiotics and antiseptic solutions are prescribed for patients in the postoperative period as well.

The method of silvering the abutment is also recommended, since silver has bacteriostatic and bactericidal properties. A number of experiments is conducted to confirm that silvering abutments and implants will allow to avoid the complications associated with infection of the implant site.
