A healthy man, 68, had a loose bridge across teeth Nos. 5 through 12. Moreover, teeth Nos. 5 and 7 gave him a tremendous pain. There were abutments on the teeth Nos. 5, 7 and 12 that supported the bridge. The conducted examination and radiograph revealed that abutments on teeth Nos. 5 and 7 have failed because of the decay. Taking into account that the teeth Nos. 5 and 7 had to be removed, there were two possible options of treatment – to remove dental implant or partial denture. Teeth Nos. 10 through 12 had no defects. With patient’s financial means, it was determined to section the bridge between teeth Nos. 9 and 10, extract the teeth Nos. 5 and 7, and insert implants in the extraction sites of teeth Nos. 5, 6, 8, and 9 with bone enlargement around the extraction sites of teeth Nos. 5 and 7.

Treating procedure

A CS 3600 intraoral scanner was implemented to make a digital impression of the current bridge and to manufacture a temporary partial denture for teeth Nos. 5 – 9. CS 9300 system (Carestream Dental) has been used for the CBCT scan to estimate the bone and implant angulation. The surgery was scheduled with the surgical template ready.

After the local anesthetization, the bridge was cut between the teeth Nos. 9 and 10. The decay was so great that the teeth Nos. 5 and 7 have broken and fallen out with the bridge. The tissue has been punched out with the help of the template and implant sites have been scalloped out to set the ovate pontics.

As the template was snapped back, the sites of teeth Nos. 5, 6, 8, 9 were drilled for implants that were placed with the template and tightened to primary stability. After that, the bone has been placed to augment the extraction sites of the teeth Nos. 5 and 7, healing caps have been placed, and the partial has been seated. Later, when the implants can be healed, the definitive crowns should be placed.

Consideration

Choosing a surgical template for dental implants led to the conservative treating procedure and required less curing time. With its help, the insertion of four implants has been conducted in minutes without disturbing the adjoining tissue. Use of CBCT made the procedure more confident, as digital technologies guarantee more precise results. Accurate and particular 3D images made by CS 9300 contributed to the better planning of the case.
